Student Registration Kindergarten - Part I

 

Kindergarten registration links below will become active in March, yearly.

Welcome to the Canon-McMillan School District

This is an exciting time for both you and your child and we hope that your child’s transition to our district will be a smooth one.

We look forward to you and your child becoming a part of the Canon-McMillan family.

Canon-McMillan only offers morning (AM) or afternoon (PM) Kindergarten.

Your child must be five (5) years old, on or before September 1st, to enroll in Kindergarten.

 

To register your child(ren), please follow these steps:

  • Click on the school link below to take you to the Kindergarten Registration form

          (Links below are only active during the month of March, annually)

  • Print and fill out these additional forms
  • Fill forms out completely, and bring to your appointment  (one per child)

  • Bring to your appointment (copies of your original forms will be made during your visit)

    • Completed forms, above

    • Original birth certificate

    • Current Immunization Records, (for Pennsylvania School Vaccination requirements click here)

      • These may either be emailed directly from your pediatrician's office or faxed to the school

    • Custody order, if applicable

    • Two (2) Proofs of Residency (a driver's license can not be used as a proof of residency)
