Athletics Links & Healthy Roster Information

Canon McMillan School District Athletic Department uses Healthy Roster to electronically collect all PIAA and AHN paperwork. Below is the required paperwork, including the PIAA CIPPE Form.

• PIAA CIPPE Sections 1-6
• AHN Release of Protected Health Information
• AHN Consent of Treatment
• PIAA CIPPE Section 7 (Only required for winter and spring athletes who have played other sports during the current school year)

If you already have an account but need to add another child you do not need to create a new account. Please message our athletic trainers through the Healthy Roster app and they will add them.
