According to Pennsylvania State Law, the following immunizations are required for attendance in school. Written physician documentation of the immunization is required prior to attendance in school.
- 4 doses of Diphtheria and Tetanus-DPT, DTaP, Td or DT (one dose must be given on or after the 4th birthday)
- 3 doses of Polio vaccine
- 2 doses of Measles-Mumps-Rubella (the first doses after 12 months of age)
- 3 doses of Hepatitis B-properly spaced
- Chicken Pox immunity-physician written documentation of 2 doses of Varicella immunization OR a signed statement from the parent or
physician as proof of having the chicken pox disease
-In addition to the above listed vaccines, students attending 7th grade will need the following:
-1 dose of the tdap vaccine
-1 dose of the meningitis vaccine
A record of immunizations is maintained for each student. If your child receives additional immunizations, please send this information in to your school nurse so the school record can be kept up to date.
