24-25 Student Registration (K-12)

To register for the current school year, please follow these steps:

1. Print this Registration Packet (one per child), fill them out completely and bring them to your appointment.

2. Schedule a registration appointment by either emailing register@cmsd.k12.pa.us, calling 724-746-2940 or visiting our District Central Office at 200 Big Mac Boulevard Canonsburg, PA 15317.

3. Bring these items to your appointment:

• Completed forms
• Original birth certificate
• Immunization records

  • These may be emailed directly from your pediatrician's office or faxed to 724-746-9184

• Custody order, if applicable

• Two (2) proofs of residency (a driver's license cannot be used as a proof of residency.) Both must have parent/guardian name.
  • Lease agreement or mortgage statement
  • Utility bill (gas, water, electric, sewage)

A child is not considered to be officially registered until all paperwork is finalized.

Change of Address
If you move within the boundaries of the Canon-McMillan School District you will be required to provide two new proofs of residency to correspond to your new residence.

Please contact your building secretary to add these documents to your child’s permanent record (see above for eligible proofs of residency.)
